New Patients At Apex Chiropractic

Your First Visit At Apex Chiropractic In Santa Monica

Your first visit is an opportunity for us to welcome you to the office and gather very important information about you and your health history. You should be prepared to be in the office for at least 90 minutes on the day of your first visit.

Upon entering the office, you will be given a “New Patient Information Form” to complete. Usually this will take less than 10 minutes. This information will be used to assess your general health as well as your chief complaint so that an accurate diagnosis can be made and an effective treatment program can be designed for you.

As soon as you have completed the New Patient Information Form, the Doctor will sit down with you and answer any questions you may have. Next he will take a careful history. At this time the Doctor will ask you several questions about your general health, past injuries and then specific questions regarding your chief complaint.

When the history is complete, he will perform a comprehensive exam. This includes taking your vital signs as well as a physical, orthopedic and chiropractic exam.

After he has collected all the necessary information, the Dr. will prepare a report of findings and a treatment “plan of action.” This is an outline of your exam findings, diagnosis, and a plan for treatment. If additional information or tests are necessary such as blood work or X-rays, you will be referred to the appropriate location to obtain this additional information.
